Important Notice for Potential NHS Athletes

BEFORE A STUDENT IS PERMITTED TO DRAW EQUIPMENT OR PRACTICE ( in any manner including pre-season and/or summer conditioning) he/she must have a health screening, insurance coverage and parental consent documentation. The health form date must be "current"---that is, not older than 365 days---and, upon expiration during any part of the participation, must be renewed before the student can continue to take part.

Both of the following forms must be completed IN FULL and on file in the activities office prior to any type of participation in a sport.
