Pathways Frequently Asked Questions

1). What steps do I need to take to fulfill my Pathways activities?
In November of each year, you will receive an email to your CCPS email address (physical letters will be sent to all employees who work in maintenance, transportation and nutrition services). The instructions will indicate:
- Bloodwork - Make an appointment with Quest to get your bloodwork and biometrics screening completed by the March 31 deadline. The registration key will be listed in your notification.
- Approximately 72 hours after your appointment you will receive an email indicating that your results are ready. Log back in to Quest to get your results, and print your lab results.
- Promptly schedule an appointment with your doctor for a “Preventative Physical exam”. Take your lab results and the Pathways Qualifier Verification Form (proof of physical) with you to this appointment.

- A Pathways Coach will reach out to you to schedule a consultation, if necessary.
- Submit the completed, signed Pathways Qualifier Verification Form (proof of physical) to the Pathways Coaches by the deadlines by faxing it to 239-377-0715 or by scanning and emailing it to CCPSPathways@CHealthPartners.com.
- Completion of your lab work and biometrics screening is NOT a physical exam.
2). Do my spouse and/or dependents have to participate in the Pathways activities?
- No. Your spouse and/or dependents will follow you on the pathway you choose.
3). What is my username and password for https://my.questforhealth.com?
- Upon registering, you will create your own username and password. These credentials do not change each time you participate in the Pathways program, so please remember them.
- If you need to reset both your username and password, first click on “Forgot Your Username”, and then “Forgot Your Password”. You will be prompted to enter the registered email address. This is your CCPS email address. The registered email address would only be a personal email address if you changed it after the first time you logged in.
4). Is the bloodwork and biometric screening paid for?
- You must schedule an appointment for your labs by going to https://my.questforhealth.com in order for these screenings to be paid for by CCPS. Do NOT schedule an appointment through Quest’s main website.
- Any costs accrued for additional lab work ordered by your doctor will be your responsibility.
5). What is a physical examination?
- The purpose of an annual physical exam is to help your doctor determine the general status of your health and focus on disease screening and prevention.
- There are no absolutes in a routine physical because everyone is different, however, an annual physical typically includes:
- evaluation of your medical and family history, and current health conditions
- review of recent lab results and other tests
- a “head to toe” physical exam
- updating immunizations, prescriptions, list of specialists and treatments
- providing counseling on a healthy lifestyle such as smoking, diet, exercise, sleep, alcohol etc.
- making sure you are up-to-date on preventive screenings
- identifying any issues that may become medical concerns in the future
- depending on your age, condition, or medical/family history recommending additional testing such as repeat blood work, bone density, EKG, X-Rays etc.

6). Do Department of Transportation (DOT) physicals count as an annual physical for Pathways?
- No. DOT physicals focus on health concerns specifically related to your ability to drive whereas a regular physical focuses on your overall health and disease prevention.
7). Is the physical examination, mammogram or colonoscopy paid for?
- The physical examination will be paid by CCPS once every calendar year. The physical examination for Pathways will be paid if the exam is solely for the preventative screening activity and the appointment is coded by the doctor’s office as a preventative screening. If any other issues or conditions are discussed during this doctor’s visit, you may be subject to a co-pay and deductible.
- Mammograms are paid for by CCPS once every calendar year after the age of 40. 3D mammograms are also paid if the 2D (standard) and 3D mammograms are billed together, not as separate services.
- Colonoscopies are paid by CCPS once every 10 years after the age of 45.

8). How often do I need to get a physical examination, mammogram or colonoscopy for the Pathways program?
- Physical Examination: Once per calendar year.
- Mammogram: Starting at age 40, once every two years*.
- Colonoscopy: Starting at age 50, once every 10 years*. Colonoscopies are covered by CCPS starting at age 45, however, the Pathways Program does not require them until age 50.
*Always follow the advice of your physician regarding your screenings.
9). When will I upgrade to the Enhanced Pathway?
- Early Upgrade - April 1: If you complete your bloodwork and biometrics screening, preventative medical screenings (i.e. physical examination, mammogram and/or colonoscopy) and wellness consultations (if necessary) by March 31, you will be upgraded to the Enhanced Pathway effective April 1.
- Mid-year Upgrade - July 1: If you complete your bloodwork and biometrics screening by March 31, then preventative medical screenings (i.e. physical examination, mammogram and/or colonoscopy) and wellness consultations (if necessary) by June 30, you will be upgraded to the Enhanced Pathway effective July 1.
- Regular Upgrade - January 1: If you complete your bloodwork and biometrics screening by March 31, then preventative medical screenings (i.e. physical examination, mammogram and/or colonoscopy) and wellness consultations (if necessary) by September 28, you will be upgraded to the Enhanced Pathway effective January 1.
10). What do I need to do to qualify for Basic, Custom or Enhanced?

11). What happens if I complete some, but not all, of the wellness incentives?
- If you do not complete your bloodwork and biometrics screenings, you will be placed in the Basic Pathway.
- If you do not complete wellness consultations and repeat A1C (if needed), you will be placed in the Basic Pathway.
- If you do complete the blood work and the wellness consultations but not the preventive screenings, you will be placed in the Custom Pathway.
12). Can I submit bloodwork results from my physician for Pathways?

No, bloodwork from your physician cannot be accepted for the Pathways Program.

Why not? Because the Pathways bloodwork and biometrics include an extensive amount of screening using lab tests and biometrics that most physicians may not include in their routine testing. The Pathways health coaches use this detailed information to determine if a Wellness Consult is needed.

Your Pathways Team strongly encourages you to take your Quest results with you to your physician so he/she can review these results during your next appointment and use this information to develop and/or continue your health and wellness goals.
13). I had a hysterectomy. Am I required to have a PAP as part of the physical examination?
- If your treating physician believes this procedure is unnecessary or you have a condition that prevents you from undergoing the screening, they must provide a note of explanation exempting you from the screening.
14). I had a double mastectomy. Am I required to undergo a mammogram?
- If your treating physician believes this procedure is unnecessary or you have a condition that prevents you from undergoing the screening, they must provide a note of explanation exempting you from the screening.
15). I have health issues and cannot undergo a colonoscopy. Am I required to have a colonoscopy?
- If your treating physician believes this procedure is unnecessary or you have a condition that prevents you from undergoing the screening, they must provide a note of explanation exempting you from the screening.
16). Do I HAVE to participate in the Pathways program?
- No this is a voluntary program.
17). I have a fear of needles. I heard butterfly needles are good for people like me. Does Quest use a butterfly needle?
- Please check with Quest at your appointment.
18). Can I request a manual cuff when Quest is performing the blood pressure screening?
- Please check with Quest at your appointment.
19). Can I choose my Pathways Coach?
- Yes. Please let your Pathways Coach know your preference when you schedule your appointment.
20). What is meant by “fasting” and how long before the blood draw should I fast?
- Nothing to eat or drink for 9 hours except water and medications.
