Being emotionally well is more than just handling stress. It also involves being attentive to your thoughts whether positive or negative.
Collier County Public Schools, Community Health Partners, and Guidance Resources have joined forces to offer CCPS employees and their families a confidential, HIPAA-compliant Emotional Wellness Program. In addition to the on-site Emotional Wellness Program provided by Community Health Partners' (CHP) team of professional counselors, CCPS also offers counseling services and work-life support provided by Guidance Resources.
Combined into one, integrated program, the CCPS Emotional Wellness Program provides you with all the services you need, whether provided on-site or at another location, whichever model most comfortably meets your needs.

Community Health Partners
CHP's program is FREE to all enrolled in the District's medical plan. Those not enrolled in the District's medical plan may be eligible to utilize the services also, and should contact CHP's Emotional Wellness Department at (239) 659- 7751 to verify eligibility.
To schedule an on-site appointment, contact Josie Means, Intake Coordinator, at (239) 659-7751 or by email at ew@chealthpartners.com. Appointments are available Monday through Saturday at Community Health Partners located at 851 5th Ave N, Suite 201, in Naples. Susie Rosbottom and Sandra Hack are available Monday through Friday at the CCPS Administrative Center located at 5775 Osceola Trail. See the CHP Team below.
Cancellations must be made at least 24 hours prior to scheduled appointment time to avoid any out of pocket costs to the member. You may cancel or reschedule your appointment:

- email ew@chealthpartners.com.
Effective July 1, 2023, the no-show fee will be increased to $50.

Meet the CHP Team:
M. “Susie” Rosbottom, LMFT
Susie Rosbottom, Licensed Emotional Wellness Clinician, has joined Community Health Partners to assist members in working through challenges including, but not limited to, relational issues, stress management, adjustments to life transitions, anxiety, depression, parenting issues, trauma, grief/loss, and substance abuse. Susie works with individuals, couples, families, groups, and all ages of children within the context of their families. Her strength-centered approach focuses on promoting emotional wellness, selfcare, healthy relationships, and an understanding of the various systems that impact her clients. M. “Susie” Rosbottom, LMFT, earned her undergraduate and graduate degrees from Stetson University in Deland, FL. She has been working in the mental health field for 20 years, with the majority of those years serving Southwest Florida. She has worked in both public and private sectors. Susie has lived in Naples since 1983. Appointments are available at the CCPS Administrative Center Monday through Thursday.
Sandra Hack, MSW
Sandra Hack, Clinical Social Work Intern, has joined Community Health Partners to assist children, adults and families work through a multitude of stressors. Sandi has extensive experience, starting her career as a certified Montessori pre-school teacher, and spending 15 years in the pediatric health care industry. After motherhood, she returned to school and graduated with Bachelor's and Master's degrees in Social Work from Florida Gulf Coast University. Sandi has extensive experience working with clients on grief and loss issues, domestic violence, depression, anxiety, severe and persistent mental illness and high-risk behaviors.
Jaime Crossan-DeBres, LCSW
Jaime Crossan-DeBres, Licensed Emotional Wellness Clinicial, has joined Community Health Partners to offer services to adults, couples, families and children of all ages that are struggling with emotional and behavioral issues. Services for adults and children will include parenting classes, child development educational classes, play therapy, age-appropriate/age-specific services, anti-bullying, healthy relationships, child anxiety management, trauma focused therapy and services for PTSD. Jaime has over fifteen years of clinical experience. She completed her undergraduate degree in Sociology and Women’s Studies from The University of Toronto and received her Master’s degree in Social Work from Florida Gulf Coast University. Appointments are available Tuesday through Friday.
Anabel Sathan, MSW (bi-lingual in Spanish)
Anabel Sathan, Licensed Clinical Social Work Intern, has joined Community Health Partners to assist members in working though issues related to depressed mood, domestic violence, anxiety, healthy relationships, grief, loss and other common stressors to get you on your path to wellness. Anabel has over five years of clinical experience, most recently in palliative care. She completed her undergraduate degree and graduate degree in Social Work at Florida Gulf Coast University. Anabel is committed to providing excellent clinical counseling and believes therapy to be an empowering tool to help individuals discover their inner strength and reach their potential. Appointments are available Monday through Friday.
Berta Garcia, LCSW (bi-lingual in Spanish)
Berta Garcia, Licensed Clinical Social Worker, has joined Community Health Partners to assist members in working though issues related to depressed mood, domestic violence, anxiety, healthy relationships, grief, loss and other common stressors. Berta has over five years of clinical experience, most recently in hospital social work and crisis intervention. She completed her undergraduate and graduate degrees in Social Work at Florida Gulf Coast University. Berta is committed to helping people find and develop their best self. She believes in solution-focused counseling that provides people with the tools, skills and training to realize their own dreams and achieve balance. Se habla Espanol.
