AFFIDAVIT OF DEPENDENT ELIGIBILITY

If adding a dependent to the medical coverage who is twenty-six (26) years of age but less than thirty (30) years of age, an Affidavit of Dependent Eligibility must be submitted to the Benefits & Wellness Office.
