• Permissible Benefit Enrollment Event*

    (For midyear changes to Medical, Dental, Vision and/or FSA coverage)

     

    What is a Permissible Benefit Enrollment Event?

    A Permissible Benefit Enrollment Event is a change in an employee’s status that allows the employee to change a pre-tax benefit election during the special enrollment period. The employee must change his or her election within thirty (30) calendar days of the event that permits the enrollment change.

    Permissible Enrollment Events:

    A. Change in the employee’s legal marital status
    B. Birth of the employee’s child
    C. Adoption of a child by the employee, provided the child is under the age of 19
    D. Employee and/or dependent involuntary loss of coverage
    E. Change in employment status
    F. Death of the employee or dependent
    G. Court order
    H. Other events may qualify (Please consult with the appropriate Benefits & Wellness contact listed below)

     How to initiate your request:

    1. Send written request to the Benefits & Wellness Office at benefits@collierschools.com and include appropriate verification document included in #2 below.

    2. Select one of the two change options below:

    If ADDING medical, dental or vision coverage due to the employee and/or dependent loss of coverage from another provider or a different Permissible Enrollment Event, follow these steps: 

    -Email written verification from employer or insurance provider confirming participant’s and/or dependent’s name(s), type of coverage and termination date to Benefits@collierschools.com.            

    -MEDICAL ONLY - Dependent documentation will be required when adding a dependent(s) to the Medical plan – Click HERE to review the Dependent Required Documentation.  Email documentation to the appropriate Benefits & Wellness contact listed below.

    -Provide Social Security Number (for security purposes, call the appropriate Benefits contact listed below).

    -Other documentation may be requested based on the type of Permissible Enrollment Event.

    -NEWBORNS
    Email newborn’s full name and date of birth to Benefits@collierschools.comOnce available, call the Benefits & Wellness Office and provide newborn’s social security number and email copy of State Birth Certificate to Benefits@collierschools.com.

    If REMOVING medical, dental, or vision coverage for the employee and/or dependent(s) due to gaining new coverage or other Permissible Enrollment Event, follow these steps:

    -Email written verification from employer or insurance provider including effective date of new coverage, participant and/or dependent(s) name(s) and type of coverage to    Benefits@collierschools.com.

    -Other documentation may be requested based on the type of Permissible Enrollment Event.


    Timeline:

    The employee must change his or her election within thirty (30) calendar days of the event that permits the enrollment change.

    If applicable, required dependent documentation must be submitted within thirty (30) calendar days from the date of the Permissible Enrollment Event notification.

    The Benefits & Wellness Office will confirm via email if the request is denied or approved. If approved, an email confirmation will be sent when the change has taken place, directing the employee to acknowledge the benefit change(s) through the CCPS Portal. Acknowledgement of the benefit change must be provided by the employee.

    Note: Benefit premiums will be prorated when approved for a midyear Permissible Enrollment Event. The final calculation is based on the number of covered days left for the calendar year.

     

    Other Changes:

    Post Tax Benefits (listed below) can be canceled at any time during the year by sending a written request to Benefits@collierschools.com.

    -Supplemental Life Insurance
    -Voluntary Life Insurance (Employee, Spouse, Child)
    -Disability
    -Worksite Benefits (Accident, Hospital, Critical Illness, Universal Life Insurance
    -Pet Insurance
    -Legal Shield & Identity Theft

    Beneficiary updates can be done at any time during the year by sending a written request with the changes to Benefits@collierschools.com.
