• Permissible Benefit Enrollment Event*

    (For midyear changes to Medical, Dental, Vision and/or FSA coverage)

    Outside of Open Enrollment, you cannot make mid-year benefit changes unless you have experienced a Permissible Benefit Enrollment event. Those life events allow you to change your pre-tax benefits elections in the middle of the plan year if certain requirements are met. 

    Changes must be requested to the Benefits and Wellness Office within 30 days of the Permissible Enrollment event. In addition to requesting the change, the proper forms and documentation need to be provided.
    Review the below list of qualifying life events for benefit changes allowed and required documentation to evidence the change.

Permissible Enrollment Events:

Marriage

    After your date of marriage, you have 30 days to make adjustments and/or changes to your benefits (Medical, FSA, Dental and Vision).

    • Permitted Changes:

      • Add spouse
      • Add children of the partner
      • Add previously eligible children if spouse is added
      • Revoke coverage
      • Enroll or increase Flexible Spending Account (FSA) election

    • Documents Required (Medical changes only):

      • Marriage certificate
      • Your most recent federal income tax return. If most recent tax return is not available, please submit the Marriage Affidavit .Tax returns must be provided when available.
      • Birth certificate, paperwork from adoption or legal guardianship
      • Social Security Number for all enrolling
      • If revoking coverage - must provide proof of other group coverage

 

Divorce

    You have 30 days after your divorce or legal separation to make adjustments and/or changes to your benefits (Medical, FSA, Dental and Vision).

    • Permitted Changes:

      • End coverage for spouse
      • Enroll in plans only if you and/or dependents lost other coverage
      • Must remove stepchildren or children of former partner
      • Decrease Flexible Spending Account (FSA) election or enroll if coverage is lost under spouse's health plan

    • Documents Required:

      • Copy of final judgment or copy of legal separation notice
      • Proof of loss of other group coverage

 

Birth/Adoption/Legal Guardianship

    You have 30 days after the birth/legal adoption of your child to make adjustments and/or changes to your benefits (Medical, FSA, Dental and Vision).

    • Permitted Changes:

      • Newborn child
      • Add newly eligible dependent
      • Add previously eligible, but not yet enrolled dependents
      • Enroll or increase Flexible Spending Account (FSA) election
      • Enroll or increase Dependent Flexible Spending Account (FSA) election

    • Documents Required (Medical Changes only):

      • Birth certificate
      • Legal paperwork from adoption or legal guardianship
      • Social Security number for all enrolling

 

Employee and /or Dependents Gaining Eligibility for Outside Coverage

    You have 30 days from the effective date of the new coverage to make adjustments and/or changes to your benefits (Medical, Dental and Vision).

    • Permitted Changes:

      • Revoke coverage for self and/or spouse and dependent

    • Documents Required:

      • Proof of other group coverage for each individual being removed

 

Employee and /or Dependents Losing Eligibility for Outside Coverage

    You have 30 days from the effective date of the loss of coverage to make adjustments and/or changes to your benefits (Medical, Dental and Vision).

    • Permitted Changes:

      • Add self/spouse/dependent who lost coverage

    • Documents Required:

      • Proof of loss of group coverage and
      • If adding dependent children (Medical Only): Birth certificate or legal paperwork from adoption or legal guardianship
          or
      • If adding spouse (Medical Only): Marriage certificate and most recent federal income tax return

 

Death of a Dependent

    If you experience the death of your spouse or dependent child, notify the Benefits & Wellness office about your loss as quickly as possible. Coverage will end effective the date of death.

    • Permitted Changes:

      • End coverage for your deceased spouse or child
      • Enroll in plans only if you and/or dependents lost other coverage
      • Decrease Flexible Spending Account (FSA) election or enroll if coverage is lost under spouse's heath plan

    • Documents Required:

      • Copy of the death certificate

 

Other

    If you believe you have experienced a qualifying Permissible Enrollment Event not listed here, please consult with the Benefits and Wellness office. We will help you determine if you have experienced a qualifying life event that allows for a mid-year enrollment change.

 

 

  • How to initiate your request

    Send a written request to the Benefits & Wellness Office at Benefits@collierschools.com and include appropriate verification document.

    The Benefits & Wellness Office will confirm via email if the request is denied or approved. If approved, an email confirmation will be sent when the change has taken place, directing the employee to acknowledge the benefit change(s) through the CCPS Portal. Acknowledgment of the benefit change must be provided by the employee.

    Premium Adjustments

    Benefit premiums are prorated when approved for a midyear Permissible Enrollment Event. The final calculation is based on the number of covered days left for the calendar year. Also, as employees have 30 days in which they can request a change because of a life event, the premium change could be retroactive and result in multiple premiums deducted from a paycheck
    .

    Other Changes

    Post-Tax Benefits (listed below) can be canceled at any time during the year by sending a written request to Benefits@collierschools.com.

    -Voluntary Life Insurance (Employee, Spouse, Child)

    -Disability
    -Worksite Benefits (Accident, Hospital, Critical Illness, Universal Life Insurance)
    -Pet Insurance
    -Legal Shield & Identity Theft

    Beneficiary changes can be done at any time during the year by sending a written request with the changes to Benefits@collierschools.com.
