Termination of Enrollment in the Home Education Program

The parent shall file a written notice of termination upon completion of the Home Education program with the district school superintendent, along with the annual evaluation, within 30 days of termination. 

Termination Procedure

Complete Termination of Home Education Program by completing the online survey.

For completion of the Home Education program, you can complete the Home Education affidavit.

If you have moved to another county in Florida and want to transfer your Home Education program to that county, you can opt to complete the Home Education Program Transfer Request form.
