Intramural Acknowledgment

Please fill out this form and return by the date provided if your child is interested in participating in the intramural as well as the tryout portion of their sport.

Aktivate Instructions

Aktivate only needs to be completed if your child was selected to participate in the official Pine Ridge Middle Interscholastic Team. Aktivate is an online platform for parents to provide the neccesasary paperwork needed prior to athletic competitions. 

Physical Exam Form 

As part of the Aktivate Clearance process once athletes are selected to their respective teams students are required to complete a Physical with a Physician once per year.
