- Data Entry gives the form to the School Nurse.
- School Nurse enters the information into FOCUS.
- Once entry is complete, the School Nurse gives the form to the Health Screening Coordinator. The hard copy of the form is kept onsite until the end of the year.
- School Nurse electronically uploads the form to DocVault-Cat B.
