Pathways to Enhanced Health

How To Participate in 2027 Pathways to Enhanced Health

Your activities for participation depend on if you were born in an EVEN or ODD birth year and what Pathway you are in as of January 1, 2027.

Pathways to Enhanced Health is the District's wellness program, which is administered by Community Health Partners (CHP). This is a voluntary program focused on promoting our employees' health and well-being through preventive care opportunities, called incentive activities.  The Pathways plan applies to all covered employees, including COBRA participants and retirees covered by the CCPS medical plan.

Incentive activities include bloodwork, biometric screening, wellness consultations, and age/gender-based preventive screenings. Employees who complete incentive activities may qualify for lower deductibles and out-of-pocket expenses.  Our goal is to support employees in reducing their health care cost by providing the resources and information to live a healthy life.  All incentive activities are designed to promote good health and minimize or manage chronic or serious medical conditions.
