This database will provide photos to multiple applications, such as the CCPS Portal, Data Warehouse, Gradebook, Student Pass, and FOCUS. *** This form must be submitted by an Administrator/Office Manager. *** Please complete and submit this form listing individual(s) at your location which may be responsible for uploading student photos.
