STUDENT RECORDS
Education Records may contain information about a student, such as: a student’s name, address, and telephone number; a parent’s or guardian’s name and contact information; proof's of address submitted at the time of enrollment; grades and test scores; health and immunization records; discipline reports; documentation of attendance; schools attended; courses taken; awards conferred and degrees earned; and special education records including individualized education programs (IEPs). 

TRANSCRIPTS
Transcripts are defined as academic records of students who attended Collier County Public Schools and are issued upon the student's request. Transcripts may be Official or Un-Official. 

OFFICIAL TRANSCRIPTS are generally required for admission to a college or university and for employment purposes.  Individuals requesting a transcript to be sent to a thrid-party, such as an employer or college/university, should consult with the third party to determine if an electronic transcript or a transcript issued directly to the student is considered as official.

UN-OFFICIAL TRANSCRIPTS lists all of a student's coursework but does not include the official seal or signature of a records representative.  Un-official transcripts cannot be released to a third party.

REQUESTING STUDENT RECORDS OR TRANSCRIPTS

If the student is under 18:  A parent or guardian must request the record or transcript and present a valid copy of their photo ID.

If the student is 18 or older:  The student must request the record or transcript and present a valid copy of their photo ID. If the individual is unable to pick-up the record or transcript, then the student must request the record or transcript, present a valid copy of their photo ID, and provide written and signed documentation, authorizing someone else to obtain a copy of their record(s) or transcript(s). In addition, the person authorized to request a copy of or pickup the record(s) or transcript(s) must also present a clear copy of their own photo ID as well.

Complete the Records Request Form, including uploading (1) clear electronic copy of your valid photo ID. See Identification section on this page for a list of acceptable IDs.

MAIL
You can submit your request via mail.

1. Complete, print, and sign the Records Request Form
2. Make a copy of your valid photo ID
3. Mail completed/signed request form and copy of photo ID to:

Apostilles authenticate the seals and signatures of officials on public documents such as birth certificates, court orders, or any other document issued by a public authority so that they can be recognized in foreign countries that are members of the 1961 Hague Convention Treaty.

How to file

Verification requests are processed on a first come, first served basis, and usually take two to five business days to complete. However, because of a high volume of requests at the start/end of each school year, verifications can take as long as one week.
 
Enrollment/Graduation verifications may include dates of enrollment, graduation date, and diploma earned. To verify courses taken and grades received, an official transcript must be ordered from the Records Department. (see above section on Student Records & Transcripts)
 
Enrollment/Graduation verifications will not be completed over the phone.  Submit your enrollment/graduation verification requests along with a signed student release of information to:

The following information should be included on your request for enrollment/graduation verification:

• Complete First and Last Name
• Former/maiden name (if applicable)
• Date of Birth
• Year of graduation or attendance
• Last school attended
• Daytime phone number and/or email where the requestor can be reached if there are any questions regarding this request.
• An exact address, email, or fax to which the enrollment certification should be delivered. 

Every state in the U.S. has official GED records of everyone who earned a GED in that state. Whether you need a copy of your own GED records, or want to verify that a job applicant truly earned a GED, here are the steps to take:

1. Determine in which state the GED credential was earned.
    
2. Check the state's education website to determine that state's requirements for records requests (Official GED Websites in the U.S.).
   
   Note:  GED (General Educational Development) testing and requests in the state of Florida are handled by the Florida Department of Education.  Official FLDOE GED Website
    
3. Have your information ready.  Most states require:

• Full name and all past last names
• Date of birth
• Social Security Number (some require only last four digits)
• Date of request
• Signature of GED holder (third party)
• Fax number or address where verification is to be sent (third party)
• Get authorization from the GED holder. (third party)
