Emergency Information Forms

Each year, parents must complete an annual Emergency Information Form. This form is available online and includes information regarding the health services available to students. Districts are required to offer parents the opportunity to opt-in for resources such as basic first aid services in the school clinic, non-academic counseling or mental health services, and any health services related to dental and eye care. 

Please complete the Emergency Information Form prior to the first day of school to ensure your child(ren)'s school(s) have your most up-to-date information. Having accurate contact information is critical for communication purposes in the event of an emergency. Additionally, every child in grades 3-12 must have a parent sign a Science Student Safety Agreement.

To complete the form, please visit the CCPS Portal and log in as a parent (do not log in using your child's information). Next, click on the FOCUS icon and follow the on-screen prompts to complete all sections of the Emergency Information Form, including Student Information, Contact Information, Health Information, and the Science Student Safety Agreement (if applicable).
