    PATHWAYS COACHES - WELLNESS CONSULT(S)                                                                  
    Your Pathways Team includes the Program Coordinator and Health Coaches. Their role is to guide and support you as you Step into Pathways and complete your incentive activities.  Your Pathways team is employed by Community Health Partners.  We can be reached during normal business hours, Monday through Friday, by calling (239) 377-0710.  We can also be reached through email at CCPSPathways@chealthpartners.com or by sending us a fax at (239) 377- 0715.

    The Pathways Team reviews your Quest blood work/biometric screening results and identifies your metabolic risk factors. Metabolic risk factors determine if you have an increased likelihood of developing heart disease, diabetes, stroke, and other health conditions.  Your Wellness Consult(s) are based on the type and number of metabolic risk factors that you may have. This is how we determine the number of Wellness Consult(s) that may be needed.  Metabolic risk factors include:

    After you complete your Quest incentive activities, you will receive an email from Community Health Partners letting you know if you do or do not need a Wellness Consult(s).

    Jennifer Ringle, RN, MSN, MSA

    Jennifer (Jen) Ringle, is a Master’s prepared Registered Nurse who has been involved with employer-based wellness programs since 1994. Her degrees also include a Master’s of Science in Administration, and undergraduate degrees in nursing. Her nursing career began in Toledo, Ohio, and continued southward to Morehead City, NC, Melbourne, FL, and finally to Naples in 2004. She worked for Naples Community Hospital (NCH) from 2004 to 2018, primarily in critical care. Jen joined Community Health Partners in 2018 as the Manager of Population Health, SmartChoice Health Coach, and Embedded Care Coordinator. She is excited for the opportunity to share her passion for health and wellness with CCPS employees as she helps to guide them towards embracing a life-long, healthier lifestyle.


    Amy Sciarrino, LPN

    Originally from the Chicago area, Amy fulfilled her childhood dream of making Naples her home 20 years ago and graduated from Lorenzo Walker Technical College Nursing Program. Amy began at Community Health Partners in 2009 and has been working in SmartChoice, Nutrition Education, and Lifestyle coaching programs since 2015.  She completed training through the American Association of Diabetes Educators and graduated from the C.H.I.P. program (now PIVIO).  Amy considers it a privilege to work with CCPS employees and looks forward to encouraging and supporting them along their wellness journey. 


    Malorie Fry, MS, RDN
    Registered Dietitian Nutritionist

    Malorie is a Registered Dietitian/Nutritionist licensed in Florida. She is Fort Myers born and raised, earning a Bachelor's degree in Dietetics and a Master's degree in Nutrition and Food Science, with a concentration in Clinical Nutrition, from Florida State University.  Malorie has been a health coach for 6 years and brings a wealth of knowledge to the table, but it's her authentic, honest, and vibrant demeanor that truly sets her apart.  She embraces the philosophy that food is fuel, medicine, and a powerful source of healing for the mind, body, and soul with the power to transform lives.  Malorie is dedicated to reshaping perceptions of healthy living, drawing from her own personal journey, educational background, and expertise.  Using a lively and holistic approach, her focus is to guide individuals toward wellness and balance through the power of nutrition.  


    Dayli Christiano

    Dayli is originally from Cuba and moved to Naples, Florida during her high school years.  She is bilingual and proficient in English and Spanish.  Health and Wellness are her passion.  Dayli believes the best life is a healthy life. Dayli loves to be active and spends most of her free time hiking, participating in races, or doing anything she can to be active and outdoors.  As the Pathways Program Manager and a health advocate, Dayli can work parallel with her passion and help navigate CCPS employees through their Pathway's wellness journey. She looks forward to hearing from you and answering any questions you may have.
