Preventive Screenings
Preventive screenings are a necessary step to obtain and/or maintain the Enhanced Pathway. Please refer to your personalized activities letter ("What do I need to complete?") and verify with your Pathways Coach which preventive screening(s) you should complete. Screening(s) must be completed by an in-network provider/facility in order to be covered at 100%.

Verification of completed screening(s) should be faxed to 239-377-0715 or emailed to CCPSPathways@CHealthPartners.com
Acceptable verification includes:
- Explanation of Benefits (EOB) from Allegiance
- Note from Physician including the employee's name, date of screening, and type of screening completed
- Copy of screening results
- Verification Form completed by your physician
*The final deadline to submit preventive screening(s) verification is 8/31/2024*
Colonoscopy (age 50 & over)Every 10 years starting at age 50.
If you had a colonoscopy in the last 10 years, you do NOT need to repeat it for this program. Simply submit verification of your prior colonoscopy.
Certain colonoscopy prep medications are covered at 100% by insurance. Click HERE for a list of approved prep medications.
Cologuard will not be accepted as a substitute for a colonoscopy.
Please share your Pathway’s bloodwork and biometrics results with your physician within a few weeks of receiving them. You can obtain your results by logging in to https://my.questforhealth.com 24-72 hours after your Quest appointment. The MyGuide to Health book will no longer be printed and mailed to you. If your physician elects to order additional lab work and diagnostic testing, it may be subject to out-of-pocket expenses.
